General Comments
NOTES: (1) DISHWASHER HAS MALFUCTIONED (AROUND THE FALL OF 2017). ALL EQUIPMENT AND UTENSILS ARE BEING WASHED IN THE THREE COMPARTMENT SINK. (2) IN ADDITION, THE HOBART COOLER MALFUNCTIONED ON NOVEMBER 20, 2017 AND ALL FOOD ITEMS WERE DISCARDED. A LIST OF DISCARDED FOOD ITEMS WAS PROVIDED. POTENTIALLY HAZARDOUS FOODS ARE NOW BEING STORED WITHIN THE OTHER COOLERS IN THE KITCHEN.

A FOLLOW-UP WILL BE CONDUCTED IN 48 HOURS (2/28/18) FOR ITEM #3--A PROCEDURE SHALL BE DEVELOPED BY THE FACILITY TO ENSURE POTENTIALLY HAZARDOUS FOODS ARE NOT LEFT AT ROOM TEMPERATURE FOR MORE THAN 4 HOURS.

PROVIDE FOR ALL COLD POTENTIALLY HAZARDOUS FOODS TO BE HELD AT 41°F OR BELOW AT ALL TIMES EXCEPT DURING NECESSARY PERIODS OF PREPARATION [750.120]. IN ADDITION, NO LABELS WERE FOUND ON THE POTENTIALLY HAZARDOUS FOOD (MILK) THAT WAS TAKEN OUT OF COLD STORAGE AND LEFT ON THE COUNTER FOR SERVICE. IF TIME ONLY, RATHER THAN TIME IN CONJUNCTION WITH TEMPERATURE, IS USED AS THE PUBLIC HEALTH CONTROL FOR A WORKING SUPPLY OF POTENTIALLY HAZARDOUS FOOD BEFORE COOKING, OR FOR READY-TO-EAT POTENTIALLY HAZARDOUS FOOD THAT IS DISPLAYED OR HELD FOR SERVICE FOR IMMEDIATE CONSUMPTION: (1) THE FOOD SHALL HAVE AN INITIAL TEMPERATURE OF 41°F (5°C) OR LESS IF REMOVED FROM COLD HOLDING TEMPERATURE CONTROL, OR 135°F (57°C) OR GREATER IF REMOVED FROM HOT HOLDING TEMPERATURE CONTROL; (2) THE FOOD SHALL BE MARKED OR OTHERWISE IDENTIFIED TO INDICATE THE TIME THAT IS 4 HOURS PAST THE POINT IN TIME WHEN THE FOOD IS REMOVED FROM TEMPERATURE CONTROL; (3) THE FOOD SHALL BE COOKED AND SERVED, SERVED IF READY TO EAT, OR DISCARDED WITHIN 4 HOURS FROM THE POINT IN TIME WHEN THE FOOD IS REMOVED FROM TEMPERATURE CONTROL; (4) THE FOOD IN UNMARKED CONTAINERS OR PACKAGES, OR MARKED TO EXCEED A 4 HOUR LIMIT, SHALL BE DISCARDED [750.153]. 48 HOURS

OBSERVED GROUND BEEF BEING IMPORPERLY THAWED--IT WAS LEFT OUT AT ROOM TEMPERATURE (FOR <4 HOURS). GROUND BEEF WAS MOVED TO THE COOLER IN THE STORAGE ROOM. PROVIDE FOR ALL POTENTIALLY HAZARDOUS FOODS TO BE PROPERLY THAWED IN ONE OF THE FOLLOWING WAYS: (1) IN A REFRIGERATOR UNIT IN A WAY THAT THE TEMPERATURE DOES NOT EXCEED 41°F, (2) UNDER POTABLE RUNNING WATER AT A TEMPERATURE OF 70°F OR BELOW, OR (3) IN A MICROWAVE OVEN (ONLY WHEN IT IS A PART OF THE COOKING PROCESS) [750.240]. IMMEDIATE/ONSITE
15  OBSERVED THE SEAL GASKET TO BE IMPROPERLY INSTALLED ON THE MILK COOLER--IT ITS NOT PROPERLY SECURED TO THE COOLING FACILITY. REPAIR SEAL GASKET [750.690]. NEXT INSPECTION
23  OBSERVED THE FOLLOWING NON-FOOD CONTACT SURFACES TO BE UNCLEAN: (1) EXTERIOR OF THE DISHWASHER, (2) INTERIOR OF THE OVEN [2X], (3)SEAL GASKET LOCATED ON THE MILK COOLER, (4) THE INTERIOR OF THE GE CHEST FREEZER, WHICH HAS A THICK COAT OF FROST. 25  OBSERVED ONE BOX OF PORTION CUPS AND ONE BOX OF HOT/COLD FOAM BOWLS TO BE STORED ON THE FLOOR WITHIN THE DRY STORAGE ROOM. PROVIDE FOR ALL SINGLE SERVICE ARTICLES TO BE STORED AT LEAST SIX INCHES OFF OF THE FLOOR IN CLOSED COVERED CONTAINERS WHICH PROTECT THEM FROM CONTAMINATION. 750.880 NEXT INSPECTION
29  OBSERVED THE PLUMBING BELOW THE 3-COMPARTMENT SINK TO NOT DRAIN PROPERLY--WATER OVERFLOWS WHEN THE SINK IS DRAINED CAUSING WATER TO SPILL ONTO THE FLOOR. PROVIDE FOR THE PLUMBING TO BE MAINTAINED IN A MANNAER THAT PREVENTS WATER FROM SPILLING ONTO THE FLOOR AND CREATING NUISANCE CONDITIONS [750.1110]. NEXT INSPECTION
36  OBSERVED THE FLOOR UNDER THE 3-COMPARTMENT SINK AND THE DISHWASHER TO BE UNCLEAN. PROVIDE FOR ALL FLOORING AREAS TO BE IN A CLEAN CONDITION [750.1220]. NEXT INSPECTION
